"Running Will Ruin Your Knees" and Other Myths We Need to Stop Believing

If you are a runner, you have almost certainly heard it from a well-meaning friend, relative, or coworker: "You know all that pounding is going to ruin your knees, right?" For decades, the idea that running automatically leads to joint destruction and arthritis has been accepted as a medical inevitability. However, modern sports science and radiographic evidence tell a completely different story.

If you love hitting the trails around Newbury Park or logging early morning road miles, we have good news: running does not ruin your knees. In fact, if managed correctly, it might be exactly what saves them.

What Running Actually Does to Your Joints

To understand joint health, we have to look at how your body responds to the physical stress of running. Your body is incredibly adaptive, and rather than simply wearing down like the shocks on a car, your tissues actually build themselves up to meet the demands you place on them.

  • Bone Density: Running strengthens your bones and significantly lowers your risk of osteoporosis by increasing bone mineral density. Furthermore, studies show that the formation of osteophytes (bone spurs) does not significantly differ between runners and non-runners.
  • Cartilage Health: Cartilage does not have its own blood supply; it relies on movement to act like a sponge, soaking up nutrients from the surrounding synovial fluid. Running enhances this blood flow to the knee joint, delivering the vital nutrients that support cartilage health without negatively impacting its volume or thickness.

The Arthritis Reality Check (By the Numbers)

The most persistent myth is that running causes osteoarthritis (OA). However, radiographic evidence indicates absolutely no significant differences in joint degeneration between runners and non-runners.

In fact, a sedentary lifestyle is actually far more damaging for your joints than a recreational running habit. When we look at the clinical data for knee and hip arthritis across different populations, the numbers are striking:

  • Sedentary Individuals: 10.2% develop hip or knee arthritis.
  • Recreational Runners: ~3.5% develop hip or knee arthritis.

This means that moderate, recreational running—even for 15 years or more, and even for individuals over the age of 50—safely promotes joint health and does not increase your need for a future joint replacement. Runners actually experience less knee pain and symptomatic OA than their non-running peers.

The High-Volume Exception

There is one important caveat in the data: the elite and high-volume runner.

While recreational running is highly protective, the risk curve changes for athletes pushing massive volume. The data shows that 13.3% of elite runners develop arthritis. Specifically, high-volume and high-intensity training—defined in the literature as running more than 57 miles (92 km) per week—may increase the wear-and-tear risk on the knees and hips.

If you are training for an ultra-marathon or logging heavy back-to-back long runs to prepare for a 100-mile race, your joints require highly specific load management, strength training, and recovery protocols to mitigate this risk.
